Does it have personal instances of dungeons/areas like the original Guild Wars or is it all shared?

The Personal Story is instanced, as well as dungeons (though you play those in 5-player parties). The open world is shared among all players, except when it becomes overpopulated. In that case, they make a duplicate of that zone for the excess players.

so basically most of the action still happens in the instances but what used to be disconnected towns is now a real overworld. is that about right?

The majority of the action I played was out in the open world. Very little is actually instanced.

Great moment: I picked up a stone in a cave and a huge cave troll came out. I got away from the troll but it wandered out into the nearby field and started killing cows. Eventually it got to a farm where a bunch of people were fighting off a bandit raid. We then had to deal with a ton of bandits as well as this super hard troll.

That all happened in the open world

When you're doing your personal story, almost all action takes place in instances. You basically walk through the open world from one instance to another.
But the personal story is just one part of the game, the other being the whole dynamic events/exploration/quests stuff which all happens in the open world. And surprisingly, you are always happy to see other players joining in, there are no downsides to having more people that I am aware of (except for performance issues).

Hah, I wandered in to the same cave and did the exact same thing. I was ahead of a group of like 5 people (all relatively low level for the area), picked up the rock wondering "what will this do," and then the troll showed up. An epic 15-minute battle with complete strangers as allies ensued, and we eventually won. It was great.

I would say very little. The gameplay has close to nothing to do with GW1. There are some nice moments of 'oh, this place has changed so much/not at all!' during the play.
