Reasons why that happens:
1- People often enter these things without planning and on impulse, because it sounds like an awesome idea at the time.
2- People find themselves busy with other things, and don't have time for it.
3- Or most of the time simply forget about it completely or stop caring enough to not even bothering going to the thread and saying they're pulling out. Or they feel ashamed of saying they're pulling out cause it makes them look stupid for promissing they'd do something in the first place.
And there you have why most forum collaborations crash and burn unless someone is REALLY on top of things and kicking people's butts.

If the issue is people simply not responding at all, you could change the rules to something like, "You have 24 hours to respond to the call for the next page, if you respond you get four more days after that, otherwise you are skipped immediately." Basically accept that people on the internet are going to be unreliable and minimize damage.
