To be honest I thought the pencil was an extended finger, part of the hand. It was wiggling and poking something...

The eye-teeth combination of the character on the bottom-left looks a bit like a skull.
But yes, nostrils are susectible to eyeness in games, generally. We are really good at seeing those eyes everywhere, you know!

I always thought that the eye was the face of a smiling man with a really long chin from the side and that the mouth was his body, the red part his arm. He was obviously navigating the rocket. It took me some years to get that it was supposed to be a face and it was the biggest WTF moment in my life.
