I'd imagine the game's challenge would be in attempting to emulate trivial day-to-day tasks, like drinking a glass of water, eating a sandwish, jogging, taking object that people offer you, all using overly complex controls composed of levers, buttons and switches, making even those simple tasks a test of skill. And all while being watched by real big people who expect you to behave completely normal, and will get suspicious otherwise. You lose if they find out your secret and you're taken to a secret government lab.

Almost, except no secret government lab.
You play as Herman Bean, born with the rare condition of congenital homunculism. Currently in your mid-forties, you stand only three and a half inches tall. Tired of being bossed around, you slaved day and night building a giant (relatively speaking) mechanical approximation of a normal-sized human.
Using this new identity, you completely reinvented yourself. Now you have it all: a beautiful wife, an expensive car and a high-paying job, but beneath this perfect exterior lies the terrible secret that you are a tiny, tiny man.
And you have a comb-over.
I thought about doing the whole 'taken to a lab for experiments' thing, but decided it's a lot better if you're mostly worried about being found out. It's sort of like being worried that your boss will discover you have a hairpiece or something, only to an extraordinary and ridiculous degree.

Why would you need to actually use the toilet? Couldn't you just go inside the bathroom and simulate the sounds to make it appear that way?

Will this be a story-driven adventure game, or more like a goal-based thing like a platformer (with story accompaniment)? I think it would be funny if, at the end, our hero decides that it is time to reveal the truth to the people whose trust he has gained, and they freak out, scream in horror, grab him and.......
...... fit him with a tiny toupee - all that he really needed all along. <3
(You needn't do that of course - I just see so many hilarious directions that this situation could go in. I can't wait to see how it turns out!)
