Before Kornel gets to mention it: Berserk.There is so much death, mutilation, torture, and rape in this manga (oftentimes from monsters or demons )... but it's an amazing book. Dammit, I clicked in this thread to say that. If you're into weird horror manga check out anything by Junji Ito. I recommend Uzumaki.

I saw a weird anime, or movie rather, by the name of memories. It came in 3 episodes, each telling a different story.
"Koji Morimoto, Tensai Okamura and Katsuhiro Otomo collaborate on one of the most imaginative anime films ever created. What originally started as production for an OAV series quickly turned into a theatrical masterpiece. The film combines together three creative animated films from each director."
The names of them are:
Memories - Magnetic Rose Memories - Stink Bomb Memories - Cannon Fodder
They're all viewable on youtube and just under an hour a piece, so I'd advise checking them out sometime.

Oh hey might as well bump this thread since I have something no-one seems to have mentioned. Gentlemen, I bring you Tokyo Akazukin aka Tokyo Red Hood. Plot overview borrowed from Wikipedia: Tokyo Red Hood is about an eleven year old girl who usually just goes by the name of Akazukin (Little Red Riding Hood). She has a sick obsession with a being known only as Mr. Wolf and wants nothing more than to be eaten by him to end her "immortal" life. Akazukin has lost all of her memories and doesn't even know who she really is. Her reason to want to be eaten by Mr. Wolf is not even known to her, instead it's a desire that is locked deep within her consciousness.
The beings of the world are mainly made up of two forces. The people who aid Akazukin in her search for Mr. Wolf, and the people who try to keep Akazukin away from Mr. Wolf. The people of the church and disciples of God try to keep Akazukin from completing her mission to meet Mr. Wolf. Their reason for doing this is because if Akazukin is eaten by the Wolf, it would bring along an end to the world and humankind as it exists in the present day. The people whose origins are from the underworld aid Akazukin to find Mr. Wolf so the fate of the world being destroyed can be fulfilled. They mainly do this discreetly and behind the shadows.
Akazukin works as an organ harvester for an Italian butcher named Marco. From there she hunts new bodies for him and all the while tries to find Mr. Wolf. Along the way she meets villains who stand in her way from fulfilling her wish to be eaten by Mr. Wolf. Definitely fits the definition of "fucked up" and it's pretty entertaining too, in all its absurdity. Well I watched Berserk on sunday It's fun how the twist on the anime is not a twist at all in the manga, which I have started reading so I dont feel like shit. HA

HA Prepare for mega-frustration when you manage to get as far as the manga currently goes. The author is releasing like what, two chapters a year? If even that, he's too busy playing Idolmaster and hanging out on Niconico.

I'm glad they dont use colours in the majority of manga. I sometimes feel ill just looking at them in black and white.
At least with Beserk it isnt like the other mangas where they tie the artists to a chair and force them to churn out fifty pages a day, and pay them peanuts so it ends up as rubbish.

I guess Junji Ito's Uzumaki is an obvious one. if you haven't read it, you should check it out. I'm not a big manga fan but that was some pretty fucked up stuff. check out his other stories like Enigma of Amigara Fault if you can, also.

Paranoia Agent: If by 'fucked up' you mean just 'odd', then this is ... decent. Broken into several episodes, each one kinda focusing on a different character, the series is about a number of people who's lives are affected by some hooligan who goes around at night assaulting people with a baseball bat. The last several episodes get into some odd psychological/metaphysical crap. Night on the Galactic Railroad: Religious propaganda masquerading as a book adaptation. It can get dull often, but it's one of the trippiest little movies I've ever seen. Elfen Lied was decent, but i really detest the visual style. It's got the most generic-looking characters you can possibly do in anime. I don't really even like anime all that much. 90% of what I watch I really just hate. I try to watch the first episode at least of whatever I can find, but unless it's a really amazing or really odd first episode, I probably won't watch any more I don't really get why people care so much about the sex and violence in anime. That's usually what turns me off of it in the first place. Had it not been for the basic situation in Elfen Lied, I would never have finished that series. I saw no mention of Ninja Scroll. More fucked up than Ninja Scroll is Wicked City, which IIRC is by the same director. It has a scene where the hero is sucked bodily into a giant vagina and then blasts his way out with a pistol.
In a similar vein is Urotsukidoji/Legend of the Overfiend. It was created by a director with an ambitious vision of an epic fantasy adventure, who was tragically constrained by the fact that he was actually supposed to be making a porno.

Damn, that Shintaro Kago stuff is fantastic! The multiplication one especially... he seems to have as much fun screwing around with the concept of a comic as much as the subject. Fun guy.. if a bit of a kellogs crunchy-nut.
Shintaro Kago definitely takes the cake. So damn gross, yet so engrossing. Junji Ito also makes some great horror as well. Parasyte features so really sick stuff near the end. Oh yea, and the main character's hand turns into a penis at some point in the first volume.
