Designer: "I've got a design idea! It's got tons of violence!"
Big Company Man: "Sound good, what exactly do you do?"
D: "I dunno."

Demakes are getting a little overdone, IMO - you might be better off going with the original "ultraviolence" theme. Still, there's plenty of potential in a MadWorld demake  |

Bood wants to focus on the violence, but in my opinion violence has ceased to be Adult a while ago when kids started playing games like God of War. Nowadays violence seems immature.

There's still a level above normal game violence.
Conscious vivisection, like in Flower of Flesh and Blood, or intestine torture, as in that Jennifer Lopez movie, are the first two to come to mind.
That said, while there's lots of red stuff in Mad World it's not actually all that graphic.

Retell the story of George Washington and the Cherry Tree, except use a chainsaw and make him a male stripper. It's educational AND sexy!

If you'll go with violence take it up a notch. Like Radix says, there's a level above videogame violence, usually related to torture.

Mmm... violence involving kids tends to be a VERY touchy subject. Could work if you were tasteful about it (and dropped the chainsaw assraping).
