Well actually the Painkiller team consisted of 10 people :D
10 people working for around two years (i think) fulltime, plus some external contractors (wiki says that currently there are 20 people plus external contractors, so i assume they used them back then too).

Thinking about the FPS' that I've actually enjoyed, I think it would be fair to say that I really enjoyed the over-the-top ones that don't take themselves seriously (and possibly have an interesting theme attached): Rise of the Triad, Shadow Warrior, Duke Nukem. It would also be fair to say that I also enjoy FPS' that add variations to what you might normally do in FPS: Riddick (sneaking around, stabbing people in the back, boxing matches), HL2 (controlling a horde of large bugs, gravity gun, etc).
The stuff that has literally turned me off even trying it out: Far Cry, Call of Duty (and sequels) and all the other 'serious' FPS' based on wars or set in 'normal' war setting such as jungles, etc.. it's just been way overdone.

I think adding a kick button transforms an ordinary FPS into a sublime one.
I think I spent like half of Dark Messiah kicking spiders off of ledges and watching them scream to their death.

Linearity.
The thing that turns me off of most FPS is the linear nature of the level design in a lot of them. I would love to see a "sandbox" type shooter. Maybe a cross between RPG and FPS (not that that isn't somewhat cliche by now lol). If it was done well though I think it could help the redundant nature of some of these games. I mean it is great to shoot things but I am tired of having my hand held while I am.
